Avant-garde Health and Titanium Healthcare, LLC have partnered to enable health systems to improve patient care, as part of the Centers for Medicare & Medicaid Services' (CMS') Bundled Payments for Care Improvement Advanced (BPCI Advanced) initiative. CatalyzeTM, the cross-continuum joint offering, is a turnkey solution that combines the power of the two companies' bundled payment expertise by marrying consulting and software with clinical services. The solution combines Avant-garde's application assistance, program evaluation and comprehensive analytics platform with Titanium's clinical services, in which clinical staff assist providers with patient stratification, case management and coordination for post-acute care.

Early results through more streamlined care, for the set of infection and cardiac bundles the health system included in their portfolio, show a high probability of reaching the CMS cap for reconciliation payments. More broadly, Avant-garde and Titanium's clients are realizing a substantial decrease in (1) length of stay, (2) post-acute care utilization, (3) 90-day readmissions, (4) inpatient and post-acute care costs and an increase in (5) physician engagement and (6) patient satisfaction.

The launch coincides with CMS' anticipated April announcement of the second and final opportunity for providers to sign up for BPCI Advanced and directly share in the cost savings generated in the program.
