It Was Never Just About Referrals: Why adapting to care coordination is a must

Wednesday, July 29, 1:00 - 2:00 pm ET

It takes a community to address health and social needs in a unified way. Yet most social needs interventions focus on individual referrals. This fundamental misalignment breeds inefficiency across community service providers over the long term and actually limits insights into the community risk factors that make social needs more prevalent, recurring, and chronic in nature.

Learn how social interventions have evolved in recent years — and why community coordination is now critical to achieving outcomes that last longer than a closed-loop referral. Hear why it’s now okay for health and social care providers to take a step back, reassess your options, and let your mission guide which community referral networks to join. Discover how organizations like yours can become leaders of referral networks, and how leaders can refocus referral management programs around community coordination.

About the SDOH MasterClass Hosts:

Rich Porcelli, LMHC is Activate Care’s Director of Community Engagement. In this role, he is a strategic advisor to community leaders across the United States as they address social determinants of health by unifying healthcare systems with community-based social services organizations.

Elise Bowman, MSW is the Accountable Health Community Project Director at Baltimore City Health Department. Elise leads Baltimore’s five-year, $4.3 million demonstration project to address unmet health related social needs through a program encompassing social needs screening, navigation, and data sharing.

A Day in the Life of a Care Coordinator: Team-Based Strategies Across Health and Social Care

August 19, 1:00 – 2:00 pm ET

Being a care coordinator working directly with individuals with complex health and social needs is no small task. Activate Care’s Brooke Sabia, a former care coordinator for long-term services and supports, is no stranger to the fragmentation and lack of coordination in healthcare and social services. These are a huge source of frustration for patients, and also barriers to delivering high-quality care and making progress in efforts to reduce the cost of care.

What tools do care coordinators have in their toolkit to do this job well? The best care coordination platforms include client tracking tools to see where people are in the health and social care system, longitudinal shared plans of care to keep all members of the care team centered around the needs and goals of their clients, and real-time alerts and notifications of critical information. When care coordination programs purchase software systems that deliver these tools to care coordinators in the field, everybody wins.

Come listen to the real-life stories of how a care coordinator survived without, and then flourished with, care coordination tools from Activate Care.

About the SDOH MasterClass Hosts:

Brooke Sabia, BA is a Junior Account Manager at Activate Care. Prior to this she spent several years in direct service, including time as a care coordinator for a Boston-based LTSS (long-term services and supports) program, providing person-centered care planning, care coordination, and advocacy for Medicaid members with complex health and social needs. She is a graduate of University of Massachusetts-Amherst.

David Fanara, MBA is Chief Revenue Officer of Activate Care. In this role, he leads sales, marketing, and account management teams. David has 20+ years of experience in healthcare technology and innovation. Inspired by the hard work and dedication of care coordinators, he advocates for the integration of health and social care in our communities.

Other topics that have been covered include sessions about how collective action is needed to bridge the health and social divide in our country. Every community in America faces a similar challenge: there are many different kinds of health, human, and social services that are available to people in need, yet no one standard way that information about these services is produced and shared. Activate Care and Open Referral explored how SDOH data standards are helping bridge the health and social divide to transform the health of our communities, in lasting ways.

Additionally, more and more states are incorporating social determinants of health in Medicaid programs. Activate Care and its experts and partners have helped webinar participants map out a strategy to move beyond the hype and understand how to integrate food and housing supports and other non-medical services in the healthcare delivery system. Hosted in partnership with Community Servings and Day Health Strategies, the webinar was attended by representatives from 40+ states across the country.
